Iron deficiency is one of the most common nutrient deficiencies worldwide. It’s often blamed on not eating enough red meat or leafy greens, but in practice, many people who eat a seemingly iron-rich diet still end up deficient.

The missing link is very often gut health.

Iron absorption is tightly regulated by the digestive system and the gut microbiome. When the gut is out of balance, a state known as gut dysbiosis, iron deficiency can develop even when intake looks adequate.

Let’s break this down in a simple, clear way.

Iron is essential for:

  • Making haemoglobin (which carries oxygen in the blood)

  • Energy production

  • Immune function

  • Brain development and concentration

  • Thyroid function

  • Healthy pregnancy and postpartum recovery

Without enough iron, oxygen delivery drops, and everything slows down.

Common Symptoms of Iron Deficiency

Iron deficiency doesn’t always show up clearly on standard blood tests at first. Symptoms can include:

  • Fatigue or weakness

  • Shortness of breath

  • Dizziness or headaches

  • Cold hands and feet

  • Pale skin

  • Hair thinning or hair loss

  • Brittle nails

  • Poor concentration

  • Frequent infections

  • Restless legs

  • Heavy menstrual bleeding

Many of these overlap with B12 and folate deficiency, which is why gut health must always be considered as a whole.

Types of Iron in Food (And Why It Matters)

There are two forms of dietary iron:

Haem iron

  • Found in red meat, liver, poultry, and fish

  • Highly absorbable (15–35%)

  • Least affected by gut inhibitors

Non-haem iron

  • Found in plant foods (leafy greens, legumes)

  • Poorly absorbed (2–10%)

  • Strongly affected by gut health, inflammation, and anti-nutrients

Traditional diets naturally prioritised haem iron, which places far less strain on the digestive system.

How the Gut Regulates Iron Absorption

Iron absorption mainly happens in the small intestine, but it’s controlled by several gut-dependent factors:

  • Adequate stomach acid to release iron from food

  • Healthy intestinal lining

  • Balanced gut bacteria

  • Proper regulation of hepcidin (the hormone that controls iron absorption)

When gut health is compromised, this system breaks down.

What Is Gut Dysbiosis? (Quick Recap)

Gut dysbiosis means an imbalance between beneficial and harmful microbes in the gut. It can be triggered by:

  • Antibiotics

  • Chronic stress

  • Ultra-processed foods

  • Refined sugar

  • Industrial seed oils

  • Infections

  • Low stomach acid

  • Long-term restrictive diets

Dysbiosis doesn’t just affect digestion; it directly interferes with nutrient absorption.

How Gut Dysbiosis Leads to Iron Deficiency

1. Inflammation Blocks Iron Absorption

Chronic gut inflammation increases hepcidin, a hormone that inhibits iron absorption and promotes iron storage.

This means:

  • Iron may look “normal” in storage

  • But is unavailable to the body

  • Resulting in functional iron deficiency

This is very common in people with IBS-type symptoms.

2. Pathogenic Bacteria Compete for Iron

Certain harmful bacteria thrive on iron and actively steal it from the host.

Your body responds by:

  • Reducing iron absorption

  • Hiding iron in storage

  • Lowering circulating iron levels

This is a protective mechanism, but it worsens the deficiency long-term.

3. Damage to the Gut Lining Reduces Uptake

Iron is absorbed through specialised transporters in the intestinal lining. Dysbiosis can damage these cells, reducing absorption efficiency even further.

This is especially relevant after:

  • Antibiotics

  • Gastrointestinal infections

  • Chronic diarrhoea or constipation

  • Long-term stress

4. Low Stomach Acid Impairs Iron Release

Stomach acid is essential for converting iron into an absorbable form.

Low stomach acid is extremely common and linked to:

  • Stress

  • Ageing

  • Dysbiosis

  • Overuse of antacids

Without enough acid, iron simply passes through unused.

Why Iron Supplements Often Don’t Work (or Make Things Worse)

Many people are prescribed iron supplements without addressing gut health first.

This can lead to:

  • Constipation

  • Nausea

  • Gut irritation

  • Feeding pathogenic bacteria

  • Increased inflammation

In some cases, iron supplementation worsens dysbiosis, making absorption even poorer over time.

Food-based iron, supported by gut healing, is far more effective long term.

Traditional Diets and Iron Status

Dr Weston A. Price observed that traditional cultures consumed:

  • Red meat regularly

  • Liver weekly

  • Bone marrow and blood-rich cuts

  • Slow-cooked stews and broths

  • Fermented foods daily

These diets supported:

  • Strong stomach acid

  • Robust microbiomes

  • Efficient iron absorption

Modern diets, by contrast, are high in iron inhibitors and low in microbial diversity.

Supporting Iron Levels Through Gut Healing

A traditional, gut-focused approach is key.

Prioritise haem iron foods

  • Beef, lamb, venison

  • Liver (75–100 g / 2½–3½ oz weekly)

  • Heart and kidney

  • Shellfish (especially clams and oysters)

Support stomach acid

  • Eat calmly

  • Chew well

  • Avoid drinking large amounts with meals

  • Include bitter foods and fermented vegetables

Heal the gut lining

  • Bone broth (250–500 ml / 1–2 cups daily)

  • Gelatin-rich stews

  • Slow-cooked meats

Restore beneficial bacteria

  • Sauerkraut

  • Beet kvass

  • Kefir

  • Yoghurt

  • Middle Eastern laban

  • Fermented carrots, turnips, and onions

Reduce iron blockers

  • Refined sugar

  • Ultra-processed foods

  • Industrial seed oils

  • Excess tea or coffee with meals

As gut health improves, iron absorption often increases naturally, without aggressive supplementation.

In Summary

Iron deficiency is rarely just about iron intake.

Gut dysbiosis interferes with:

  • Iron absorption

  • Iron regulation

  • Iron availability

  • Iron tolerance

Healing the gut restores the body’s natural ability to manage iron safely and effectively, the way it was designed to.

Iron deficiency is a gut issue far more often than it’s a dietary one.
